For DC Residents

Required Documents:

  1. Choose ONE of the following:
    • Healthcare practitioner recommendation (issued within 2 years)
    • Self-certification (if 21 or older)
  2. Current 2×2 face photo
  3. Choose ONE of the following ID combinations:
    • DC DMV REAL ID OR
    • Proof of DC residency PLUS government-issued photo ID

Application Process

  1. Gather all required documents before starting
    • Applications cannot be saved mid-process
    • All documents must be ready for upload
  2. Submit your application
  3. Review Process
